DICTATE: Your name (no need to spell), patient name (spell first and last), patient DOB (optional), DOS, and ESO incident/trip number. Example: Jodi Peterson, dictating for patient Gladys Schmendl, DOB 11/1/1950, DOS 12/1/22, incident number 12345.

Yellow-highlighted text will be transcribed according to what is dictated, and highlights example language to be considered. Green-highlighted text may require field not dictated. Otherwise, text not dictated will be removed.

DISPATCH: Medic X and Engine/Tower Y were dispatched by South Metro Fire Emergency Communications for a reported type of incident. Located at a private residence, commercial structure, assisted living facility, public place, etc.

RESPONSE: Units dispatched responded emergently/non-emergently, and without delays. Add any details that may have affected response or access: weather, staged by law, crew member indisposed, forcible entry required, etc.

ARRIVAL: Upon arrival, found a age-year-old male/female patient. Patient found position-sitting/standing/lying at location: bed, chair, floor, ground, vehicle, first floor apartment, second floor room, etc. The patient was complaining of what the patient says is wrong. Chief complaint/event that resulted in 911 call. If patient unable to articulate problem or PR initiated 911 call, include subjective details, scene assessment, history of presenting complaint, associated complaints, and patient/bystander/family/staff statements.

PERTINENT NEGATIVES: The patient denied LOC, headache, change in vision/hearing, dizziness, shortness of breath, nausea, vomiting, diarrhea, chest pain, abdominal pain, neck pain, back pain, numbness/tingling, paralysis, pregnancy, alcohol, drugs, recreational, illicit and legal, recent trauma, and recent illness.

ASSESSMENT: An ALS assessment was performed. All systems in assessment are required fields. If body system not assessed, state not assessed. Unremarkable, normal, and negative are not acceptable as stand-alone assessment findings. GENERAL APPEARANCE: MENTAL STATUS: SPEECH: SKIN: HEENT: NECK: CHEST: LUNGS: ABDOMEN: GI/GU: CTLS: BACK: PELVIS: EXTREMITIES: NEUROLOGIC:

IMPRESSION: Provider working diagnosis or impression of the patient's complaints or symptoms. Examples: cardiac chest pain with evidence of STEMI, respiratory failure, hypoxia, multi-system blunt trauma, altered mental status, fall with consideration for hip fracture, etc.

TREATMENT: Resulting treatments, indications, and responses to treatments. ECG exam reveals rhythm, ectopy, 12 lead findings, ST elevation, axis, and blocks. Blood glucose, tympanic temperature, lactate, pulse oximetry, end-tidal CO2, capnography. IV successful/unsuccessful, bloods drawn, type, and total volume given. Medications: amount, route, response, and reference card/Handtevy use.

TRANSPORT: Method and level of assistance needed to transfer patient to pram and from pram to hospital bed. Patient moved from point of contact by mega-mover, scoop, carried, walked. Patient transported emergently/non-emergently to hospital. Notification en route and alerts called or not called. Patient care and report transferred to receiving hospital nurse, doctor, or staff. Belongings left at bedside, with family, etc. No further actions were taken.
